Terms and Conditions

We, the registrant and the registrants legal parent or guardian, hereby agree and acknowledge the following:

(1) We agree to abide by the rules of Master Sports(Master Concepts LLC) and its affiliated organizations and sponsors.

(2) We recognize the inherent risk of serious or permanent physical injury and possible death associated with youth sports activities and games. In consideration for Master Sports accepting the youth player’s registration and participation in its sanctioned leagues, tournaments and activities (“Youth Programs”), we hereby release, discharge and/or otherwise indemnify and hold harmless Master Sports, its affiliated organizations and sponsors, volunteers, their employees and associated personnel, including the owners of fields and facilities utilized for the Youth Programs, against any claim, lawsuit or written demand, including but not limited to any claims for personal or physical injury or death, by or on behalf of the registrant as a result of the registrant''s participation in the Youth Programs and/or being transported to or from the same, which transportation we hereby authorize.

(3) We consent to emergency medical care prescribed by a duly licensed Health Care Provider or Dentist. This care may be given under whatever conditions are necessary to preserve the life, limb or registrant''s well-being and we hereby agree to be financially responsible for all costs associated with such treatment.

Refund Policy:

Refund requests must be made at least two weeks PRIOR to the start of a CLASS or CAMP. No refunds will be given inside the two week deadline. Only credit will be given in case of injury or illness, for which proof must be provided. Summer camp credit is only valid towards another summer camp that same year. All approved refunds will incur a $20 processing fee per registration. Refund requests must be emailed to: service@mastersports.com

League refund requests must be made no later than one week PRIOR to the assessment date. No league credits will be given after the assessment date, or after child has been placed on a team - No Exceptions. All approved refunds will incur a $20 processing fee per registration. Parent Code of Conduct Policy:

Parents and participants must conduct themselves in a respectable manner and any disruptive behavior is grounds for removal from the program. I hereby pledge to live up to this agreement as a Parent / Guardian of a Youth involved in a Youth Sports League / Program / Activity. Parent Expectations and Code of Ethics: [A] I will place the emotional and physical well being of my Child ahead of a personal desire to win. [B] I will Respect all persons involved in this program/activity. I will remember that I am a model for all youth. [C] I will lead by example and respect all parents, players, referees, coaches and league officials by: (1) Speaking words of positive encouragement in the presence of children. (2) Practicing basic self control and refrain from doing or saying anything negative in the presence or earshot of children. (3) Informing coaches, referees and league representatives of disagreements only before or after games and practices and in private. (4) Being responsive to the direction of coaches, referees and league officials. (5) Collaboratively resolving conflicts and disagreements with the intention of strengthening relationships with players, parents, referees, coaches and league officials. (6) Creating an alcohol, tobacco, and drug free environment during practices and games (7) Contributing to the development of safe physical and positive psychological environment for all. (8) Supporting the vision of: Competing and Winning with Honor / Play your Best, Play Fair , Every person wins. [D] I will be knowledgeable in the rules of each facility /activity my child attends, and I will go over these rules with my Child. [E] I will remember that I am a youth sports Parent / Guardian, and that the game is for children and not adults.

We consent to Master Sports taking photographs, video recordings, and/or sound recordings in documenting the activities of Master Sports programs and services. We hereby grant Master Sports and their affiliates’ permission to use the negatives, prints, motion pictures, video/audio tapings, or any other reproduction of the same for Master Sports and its affiliates’ educational and promotional purposes in manuals, on flyers, the internet, or other publications.

We have read this release and waiver of liability and fully understand its terms. We understand that we waive substantial rights by signing this form. We agree to waive all such rights above including the right to file a legal action or assert a claim for personal or physical injury or death of any kind. We sign this release form freely of our own free will.
